*You will receive an invoice for payment via email; payments are due by the first of the month.

Photo & Media Release Consent

I herby give consent to Beyond The Books to photograph, videotape, or otherwise record the image and/or sound of myself (or my child) to use for public media, Facebook, Instagram, or website use. I understand that I will not receive any monetary compensation and release the organization from any liability arising from the use of this material.

Supervision & Liability Disclaimer

Beyond the Books is an enrichment program and does not provide childcare or supervision services. Parents and legal guardians are responsible for the supervision, safety, and behavior of their child(ren) at all times while on campus or participating in any Beyond the Books activities or events. By enrolling in this program, parents and legal guardians agree to indemnify, defend, and hold harmless Beyond the Books, its board members, staff, volunteers, and affiliates from any claims, damages, injuries, losses, or liabilities that may arise from participation in classes, events, or presence on the premises.
